SBA 504 Application and Forms
GEODC thanks you for considering a Small Business Administration 504 Loan. We
believe you will find this financing to be a tool to help your business prosper
and grow. We look forward to working with you.
Our application forms have been designed to help us expedite your financing
request. When you complete the application in full we can maximize the use of
our customized software and reduce your paperwork throughout the remainder of
the process.
To begin, simply download the first of the attachments below, “504
Application”. The subsequent documents are separated for your convenience. For
example, you may want more than one copy of the Personal Financial Statement or
Resume forms, depending on the number of individuals affiliated with your
business. Completed applications can be sent to GEODC’s loan officer.
It is important for you to know this funding is Federal and therefore federal
law requires all Certified Development Corporations (such as GEODC) to obtain,
verify and record information that identifies each person who applies for a 504
loan. What this means to you: When you apply for a 504 loan, we will ask for
your name, address, date of birth and other information that will allow us to
identify you. We may also ask to see your driver’s license or other identifying
documents.
